What do you need?

- Medical documents attesting to your eligibility to secure a med card such as chronic pain conditions
- Able to consent to certification or have a proxy who can consent for your med card
- New York State residency

If you have a diagnosis of one of the following conditions you may be eligible to participate in the NYS Med Card Program
- Chronic Pain
- PTSD
- Cancer
- HIV/AIDS
- Amyotrophic Lateral Sclerosis (ALS)
- Parkinson's Disease
- Multiple Sclerosis (MS)
- Spinal Cord Injury
- Epilepsy Inflammatory Bowel Disease
- Neuropathy
- Huntington's Disease
- Substance Abuse Disorder
